Wireless providers pole license and permit
Follow the steps below to apply for a wireless facility permit, including required agreements, application materials and the review process.
How to apply
Step 1: Review documents

Step 2: Review and prepare agreement
If applicable, review the template and submit any requested immaterial changes for City review.

Step 3: Schedule appointment and submit your application
Applications are submitted electronically with an approved appointment time.
Contact the Public Works Department at 562-866-9771, ext. 2504 to schedule or email PW@lakewoodca.gov.
Submit a partially executed PLA if applicable, the appropriate application submission, site plans, construction drawings and all other required documents.

Step 4: City review and decision
The City will review your application and notify you of the decision.
Step 5: Finalize and proceed
Once approved, obtain any required permits. The City will finalize the license, and you may proceed with installation.
