Civil Rights Notice
The City of Lakewood is committed to ensuring that all individuals have equal access to its programs, services, and activities.
In accordance with Title VI of the Civil Rights Act of 1964, the City does not discriminate on the basis of race, color, or national origin.
The City also prohibits discrimination on the basis of sex, age, disability, religion, medical condition, marital status, sexual orientation or other protected status under applicable laws.
The City is committed to:
- Providing equitable access to programs, services, and activities
- Ensuring meaningful access for individuals with limited English proficiency (LEP)
- Promoting full and fair participation in City programs and decision-making processes
Language assistance is available upon request.
How to File a Complaint
Any person who believes they have been discriminated against in a City program, service or activity may file a complaint.
Complaints must be submitted within 180 days of the alleged discrimination.
The preferred method for submitting a complaint is the City's online Civil Rights complaint form (see sidebar for link).
If you are unable to use the online form, complaints may also be submitted by mail or another accessible format upon request:
