If you need assistance completing a request form, please contact the City's ADA Coordinator at  ADACoordinator@lakewoodca.gov or 562-866-9771, extension 2601.

Requests will be handled as quickly as possible. Submitting requests in advance helps allow time for coordination, but it is not required.

Our Commitment

We are working to ensure our website and digital content conform to the Web Content Accessibility Guidelines (WCAG) 2.1 Level AA, as required under Title II of the Americans with Disabilities Act (ADA).
