DASH Application
The city’s DASH transit service is free to eligible Lakewood residents age 60 or older who do not have their own transportation and to eligible residents with disabilities of any age.
DASH is a reservation-only service that travels in and around Lakewood. Eligibility requirements apply, and riders must be approved before using the service.

NOTE: A Physician’s Verification Eligibility form must be completed by a licensed physician if the applicant is under 60 years of age with a disability or older than 60 years of age with a valid driver’s license.

Complete and sign Parts I, II and III of the form.
Have your doctor complete Part IV, Physician’s Verification of Eligibility, if you are:
- Under age 60 and applying based on a disability.
- Age 60 or older and have a valid driver’s license.
Print, sign and submit the completed form by:
- Mail to: City of Lakewood, DASH Transportation, 5050 Clark Ave., Lakewood, CA 90714
- Email: Scan the completed form and email to DASH@lakewoodca.gov.
- In-person visit: Bring the completed form to the DASH office at Palms Park, 12305 207th St.
