The city’s DASH transit service is free to eligible Lakewood residents age 60 or older who do not have their own transportation and to eligible residents with disabilities of any age.
DASH is a reservation-only service that travels in and around Lakewood. Eligibility requirements apply, and riders must be approved before using the service.

How to apply
- Open the DASH application form(PDF, 556KB).
- Complete and sign Parts I, II and III of the form.
- Have your doctor complete Part IV, Physician’s Verification of Eligibility, if you are:
- Under age 60 and applying based on a disability.
- Age 60 or older and have a valid driver’s license.
- Submit the completed form by mail, email or in person.
